Pathologies of the kidney: what are positive and negative Giordano's sign
The Giordano manoeuvre is a manoeuvre used in medical semeiotics to investigate the presence of kidney pain; it is named after the physician Davide Giordano, who was active in the first half of the 1900s
How the Giordano manoeuvre is performed
- the patient is in a sitting position, with the trunk slightly flexed forward;
- the doctor stands behind the patient and, with the ulnar edge of his hand, strikes with a sharp blow the patient’s renal loggia, i.e. the point where the kidney is located, situated in the lumbar region;
- if this manoeuvre causes violent pain in the patient, the Giordano sign is called positive, otherwise it is called negative.
What does the positive Giordano sign indicate?
A positive Giordano’s sign indicates the probable presence of a kidney stone.
Is the Giordano sign sufficient to make a diagnosis?
No. As is often the case in semeiotics, the positive Giordano sign is an indication of diagnosis and is NOT sufficient on its own to make any precise diagnosis: the latter must in fact be investigated by continuing with diagnostic laboratory tests (haematochemical tests) and imaging tests (ultrasound, X-ray, etc.) that can avoid false positives and negatives.
